When it comes to care options at Plainfield Health Care Center, we don’t believe that proper care can be delivered with a generic, one-size-fits-all formula. From your first consultation forward, our experienced team will take the time to get to know you, discover your preferences and understand your concerns to create a personalized Care Plan just for you and your individual needs. To accommodate a wide variety of needs, we currently offer three different Living Options including Rehabilitation, Short-Term or Long-Term Nursing Care, and Memory Care. Within the Living Option selected, we’ll continue to tailor and customize every facet of your personalized Care Plan by adapting to your needs as they change and helping you reach one goal at a time.

24 Hour Skilled Nursing Care

Plainfield Health Care Center offers short-term or long-term options to help your loved one recovery as quickly as possible so they can return home and to their day-to-day life. We offer inpatient and outpatient therapy and rehabilitation services to all families.

Following surgery, or injury or illness, our staff provides skilled nursing services, and therapy and rehabilitation your loved one needs. There are a number of reasons you may need our care including stroke, broken bone, or surgery—our facility provides individuals with the 24 hour support they need to allow for a faster and improved long term recovery.

Housekeeping/Laundry Services

We understand that the aging process or illness can strip independence away from our residents and we strive to help our residents live as independently as possible while maintaining their security and safety. We ensure that our residents remain active in their day to day life and ensure that they are involved as much as possible in their care plan.

Each resident is given the privacy of their own space to call home with tons of extra space for them to relax and enjoy themselves in. There are additional services available at the facility such as housekeeping, laundry services, transportation, a pharmacy, and more.

Wound Care

A full scope of specialized care and treatment for post-op and pressure wounds, evaluated by a nurse practitioner.

Plainfield Health Care Center offers a comprehensive range of treatments to aid in the healing of chronic wounds caused by diabetic ulcers, pressure ulcers, radiation injuries, venous insufficiency and other conditions. We provide full-service care to treat patients with emergency and chronic wounds resulting from various health conditions.

Physician, Physician Assistant, Nurse Practitioner Services

Providing home care is a team effort from professionals, paraprofessionals, patients, and families.  Our goal is to promote the physical and emotional well-being of our clients by preserving their rights and independence. At Plainfield Health Care Center, we are the rehabilitation experts. Our facility was designed and recently remodeled to maximize patient independence for residents in our Stepping Forward program. In addition to some of the most advanced physical therapy equipment and our state-of-the-art rehab gym, our facility now boasts a total of 22 private rehabilitation suites.
